STATE (where things are, exact paths and real output), GOAL (what done looks like), CONSTRAINT (what's off the table and why), DEAD (what already failed, and how), ASK (the single next decision).

The block everyone skips

DEAD. Without it, every session proposes the thing that already failed, with enthusiasm, and you accept it because it sounds new. Paste the error, not the adjective — 200 characters of stderr transmit everything “it's broken” doesn't.

The meta-rule
